The Middletown Police Department, working together with families and caregivers for people suffering from Alzheimer's, has an information gathering form that is used to assist you. The form can be downloaded from this site or it can be obtained at the police station. The completed form along with a photo of the patient is kept on file at the front desk of the police station and it is used to provide officers with information useful in locating a person who is lost.
